At Judith Crowell, MD and Associates we have a secure Certified Electronic Medical Records system for our patients enabling them to register on-line prior to your appointment. Call our office to receive your secure access code to register or update your Medical History. This helps to eliminate any in-office delays on the day of your visit. We will notify you if your insurance company requires any referrals so you can arrive with the proper forms in hand.
While the office accepts many insurance plans, please verify our affiliation with your specific coverage when you call for your first appointment, and make sure to bring your insurance card.
If you cannot make your scheduled appointment, please call and cancel 24 hours in advance. Missed appointments, without prior notification, may result in a charge. It is standard policy that a deposit be required for complicated or time-consuming cosmetic or surgical procedures.
